Is any one aware of any resources (book, journals, other publications etc.) that investigate the failures of carrying out effective and thorough due diligence prior to the undertaking of M&A activity?

It would be both helpful and interesting to learn from real life examples and case studies, how and why due diligence processes failed to uncover costly risks/issues that were only realized post transaction.
